Patient Referral Program

We appreciate our patients, and we're proud to say that many of our new patients have been referred to us by our current patients.  So as a special way to say thank you, we are introducing a new referral rewards program to all our patients.  It's easy to be rewarded when you refer your friends and family to our practice!  Just refer a friend or family member, and be sure to tell them to write down your name as the referral source on their new patient paperwork at their first visit. 

As soon as your referral completes his/her first visit, you will receive a $25 gift card to Pyro's Pizza, Malco, Goulds, or Target.  The more referrals you make, the more gift cards you will receive!

Please Note:

• There is no limit to the number of referrals.  Limit of one gift card per immediate family when referring your spouse or children.
• New patients in the same household count as one referral.
• "New patients" are defined as those who have never been a patient at Ty H. Barkley, DDS & Associates.
• To receive the referral award the new patient must provide your name on their first visit.
